Please fill out the form below to sign up for the 2014 CSA. Then make a payment to Cully Neighborhood Farm, 5620 NE Alberta St, Portland OR 97218. We accept cash and check. Please contact me to make arrangements for cash payment: matt@cullyneighborhoodfarm.com or 503-310-5766.
If paying in full, the amount is $616 ($600 if a returning member paying by April 15th). If paying in installments, the deposit is $166 ($150 if a returning member paying by April 15th), with 3 additional payments of $150 spaced throughout the season. If signing up after the CSA season begins (5/19), payment will be reduced by $22 per week missed from the start. Payment must be received within 5 days of signing up in order to guarantee your spot in the CSA. Thank you!! After receiving your signup, you will be emailed a contract for you to complete and return.
